がんは非可逆的な異常の蓄積により発生する。エピジェネティック異常が突然変異同様に発がんに重要であることは広く知られるが、異なる性質も多い。まず、エピジェネティック異常は発がん以前から多くの細胞に存在する。その蓄積量を用いることで発がんリスク診断が可能である。また、エピジェネティック異常の誘発には慢性炎症が重要で、変異原や紫外線・放射線が重要な突然変異とは異なる。さらに、DNAメチル化異常は特定の遺伝子に誘発されることが知られ、ランダムに誘発される突然変異とは異なる。最も重要な違いは、DNA脱メチル化剤やヒストン脱アセチル化酵素阻害剤によりエピジェネティック異常は元に戻せることである。長期の副作用を避けるためには、あるエピジェネティック修飾が生理的に存在するゲノム領域への影響は最小限にして、異常に存在する領域にのみ作用させることが必要である。近年のエピゲノム解析は、特異性をもたらすためのヒントを与え始めている。

「ボリノスタットによるリンパ腫の治療」

ボリノスタット(スベロイルアニリドヒドロキサム酸)は、クラスIおよびIIのヒストン脱アセチル化酵素(HDAC)の低分子阻害剤で、遺伝子発現制御により癌細胞の細胞周期停止およびアポトーシスを促進する。ボリノスタットは様々なタイプの血液学的悪性疾患に対し、単剤治療、または併用レジメンにおいて作用を示す。日本では皮膚T細胞性リンパ腫の治療に既に承認されている。
濾胞性リンパ腫(FL)など、B細胞性非ホジキンリンパ腫(B-NHL)のサブタイプの一部では、ヒストンアセチル基転移酵素(HAT)をコードするCREBBP遺伝子およびEP300遺伝子における頻繁な体細胞変異が検出される。この体細胞変異によるヒストンのアセチル化活性の欠如により、HDACとHAT間の活性不均衡が生じる。これらの報告によると、FL患者の41%でCREBBP遺伝子およびEP300遺伝子に体細胞変異および/またはゲノム欠失が生じるとともに、アセチル基転移酵素活性が低下している。ボリノスタットのようなHDAC阻害剤は脱アセチル化活性を阻害することによりバランスを是正し、抗腫瘍効果をもたらす可能性がある。 本講演では、悪性リンパ腫に対するボリノスタットの臨床試験結果を概説した。

Cancer develops by the accumulation of irreversible alterations. Although epigenetic alterations as well as mutations are widely known to be important in carcinogenesis, epigenetic alterations differ from mutations in many respects. First, epigenetic alterations are present in many cells far in advance of the onset of carcinogenesis. Measurement of their accumulation levels enables us to diagnose the cancer risk. Secondly, chronic inflammation is an important inducer of epigenetic abnormalities while mutagens, ultraviolet, and radiation are important inducers of mutations. Third, aberrant DNA methylation is known to be induced in specific genes, unlike mutations, which are induced in mostly random genes. The most important difference is that epigenetic alterations can be reversed by the use of DNA-demethylating agents and histone deacetylase inhibitors. To avoid long-term adverse effects, it is necessary to ensure that these agents have minimal effects in the genomic regions where physiological epigenetic modifications exist, and act only in the regions where abnormal epigenetic modifications exist. Recent epigenomic analyses have begun to provide clues to the generation of specificity in these agents.

"Treatment of malignant lymphoma by histone deacetylase inhibitor"

Vorinostat (suberoylanilide hydroxamic acid) is a small molecule inhibitor of class I and II histone deacetylase (HDAC) enzymes that promotes cell cycle arrest and the apoptosis of cancer cells by regulating gene expression. Vorinostat shows activity against various types of hematological malignancies as a monotherapy or in combination regimens. It has been already approved for cutaneous T cell lymphoma in Japan.
In several subtypes of B-NHL including follicular lymphoma (FL), frequent somatic mutations in the CREBBP and EP300 genes, which encode histone acetyltransferases (HATs), are present in primary tumors and are acquired at the time of relapse. The mutation-induced loss of the acetylation of histones and other transcription factors causes an imbalance between HDACs and HATs. According to these reports, 41% of FL patients exhibited somatic mutations and/or genomic deletions in the CREBBP and EP300 genes as well as decreased levels of acetyltransferase activity. HDAC inhibitors, such as vorinostat, offer a very promising anti-tumor effect by normalizing acetylation activity.
Here I will review the results of clinical trials of vorinostat for malignant lymphoma.

Prof. Ushijima, Prof. Ando and the Chairman, Prof. Saya held a panel discussion in which the participants of the meeting also took part. At this discussion, increasing expectations for epigenetic therapeutic agents, which have begun to be practically used, could be perceived.
